ADC Program and CMS GUIDE Model
The Centers for Medicare & Medicaid Services announced on July 31, 2023, the Guiding an Improved Dementia Experience (GUIDE) Model, an alternative payment model aimed at improving the quality of life for persons living with dementia (PLWD), reduce strain on caregivers, and help PLWDs remain in their homes and communities through a package of care coordination and management, caregiver education and support, and respite services.
The Alzheimer’s and Dementia Care (ADC) Program aligns perfectly with CMS’s voluntary GUIDE model, and its requirements to improve the support of people living with dementia and their unpaid caregivers.
