Partnership with The John A. Hartford Foundation
The UCLA Alzheimer’s and Dementia Care Program has shown remarkable results for people living with Alzheimer’s and other dementias in addressing critical health care, social service, and family caregiver needs while improving health outcomes and health care costs. Through The JAHF, UCLA has been granted two grants to spread the ADC model across other health systems in the country.
Dissemination grant (2019 - 2021):
Implement the ADC Program model in health systems outside of UCLA
Work with the Gerontological Advanced Practice Nurses to educate Advanced Practice Providers to fulfill the Dementia Care Specialist (DCS) role
Work with partners to promote payment changes
Sustainability grant (2022 - 2024):
Create an ADC Dissemination Center at UCLA
Create a National Dementia Care Learning Collaborative
Continue to implement the ADC Program in additional health systems
