Implementation Guide for Adopting Sites


The purpose of this guide is to facilitate the implementation of the Alzheimer’s and Dementia Care (ADC) program in healthcare settings. The guide has two main components:

  • Structure and Operations

  • Business Planning, Evaluation, and Spread

Access to the materials within the implementation guide requires a Letter of Agreement and contract between UCLA and the adopting organization.
